Streszczenie

Wstęp

Wrodzona przepuklina przeponowa (CDH) to rzadka wada wrodzona, obarczoną wysoką śmiertelnością, wynikającą z hipoplazji płuc i nadciśnienia płucnego. W celu optymalizacji wyników leczenia ważna jest diagnostyka prenatalna, stwarzająca możliwość wyboru dalszego postępowania.

Opis przypadku

Noworodek płci żeńskiej urodzony w 36 Hbd. z prenatalnym rozpoznaniem CDH prawostronnej. Noworodka zaintubowano i wdrożono wentylację oscylacyjną o wysokiej częstotliwości (HFOV). Z powodu narastającej hipotensji podano katecholaminy. W badaniach obrazowych nie było możliwe jednoznaczne określenie lokalizacji ubytku. Pacjentkę zakwalifikowano do operacji w 4. dobie życia (dż.), po uzyskaniu stabilizacji stanu ogólnego. Śródoperacyjnie stwierdzono prawdziwą CDH, worek przepuklinowy tworzył wspólną ścianę z osierdziem. Ubytek w centralnej części przepony, sięgający od mostka do tylnej ściany klatki piersiowej został zamknięty bez użycia łaty. Okres pooperacyjny niepowikłany. W 25. dż. pacjentkę wypisano do domu w stanie ogólnym dobrym.

Wnioski

W przypadku przepukliny przeponowej, diagnostyka prenatalna i postnatalna ma istotny wpływ na sposób postępowania i rokowanie. Kwalifikacja do operacji powinna nastąpić po uzyskaniu stabilizacji krążeniowo-oddechowej.

Abstract

Introduction

Congenital diaphragmatic hernia (CDH) is a rare birth defect with a high mortality rate due to pulmonary hypoplasia and pulmonary hypertension. In order to optimize treatment outcomes prenatal diagnosis is important, providing the opportunity to choose further management.

Case Report

A female newborn born at 36 Hbd. with a prenatal diagnosis of right-sided CDH. The neonate was intubated and high-frequency oscillatory ventilation (HFOV) was implemented. Catecholamines were administered due to increasing hypotension. On imaging studies, it was not possible to unequivocally determine the location of the defect. The patient was qualified for surgery on the 4th day of life (j.), after stabilization of the general condition was achieved. Intraoperatively, a true CDH was found, the sac hernia sac formed a common wall with the pericardium. The defect in the central part of the diaphragm, extending from the sternum to the posterior chest wall was closed without the use of a patch. The postoperative period uncomplicated. The patient was discharged home in a generally good condition on the 25th of jan.

Conclusions

In the case of diaphragmatic hernia, prenatal and postnatal diagnosis has a significant impact on the management and prognosis. Qualification for surgery should be made after stabilization of the cardiopulmonary function.
