Streszczenie
Wstęp
Od wstrząsu, przez zagrażającą życiu posocznicę, do gry w piłkę w wieku 74 lat — historia pacjenta po obustronnej
kapoplastyce bioder. Kapoplastyka stawu biodrowego stanowi alternatywę dla całkowitej endoprotezoplastyki
u pacjentów aktywnych fizycznie. Pozwala na zachowanie zasobu kostnego bliższego końca kości udowej i biome‐
chaniki stawu poprzez pokrycie, a nie resekcję głowy kości udowej, nie zachowuje jednak naturalnej powierzchni
stawowej, a jej zastosowanie wiąże się z określonym ryzykiem powikłań.
Opis przypadku
59‐letni mężczyzna, dotychczas aktywny fizycznie, zgłosił się z powodu nasilonych dolegliwości bólowych obu
bioder. W 2010 roku wykonano kapoplastykę prawego stawu biodrowego. Okres pooperacyjny był powikłany
znacznym bólem oraz epizodem wstrząsu po autotransfuzji. Rok później przeprowadzono analogiczny zabieg po
stronie lewej – rekonwalescencja była szybka i początkowo niepowikłana, jednak w 3. dobie, zgodnie z ówczesną
dokumentacją, postawiono kliniczne rozpoznanie posocznicy. Pobrano posiewy krwi i wymaz z rany, a antybio‐
tykoterapię następnie skorygowano według antybiogramu; tożsamości drobnoustroju nie da się już odtworzyć
na podstawie dokumentacji pozostałej u autora. Według stanu na rok 2026, w wieku 74 lat, pacjent pozostaje
aktywny fizycznie.
Wnioski
Pomimo ciężkich powikłań wczesnych, odległy wynik funkcjonalny może być bardzo dobry. Kluczowe znaczenie
ma właściwa kwalifikacja pacjenta oraz szybkie leczenie powikłań.

Abstract
Introduction
From shock to life‐threatening sepsis and back to playing football at the age of 74 — the story of a patient after
bilateral hip resurfacing. Hip resurfacing is an alternative to total hip arthroplasty in physically active patients.
It preserves femoral bone stock and joint biomechanics by capping rather than resecting the femoral head,
although it does not preserve the native articular surface and is associated with a specific risk of complications.
Case Report
A 59‐year‐old physically active man presented with severe pain in both hips. In 2010, right hip resurfacing
was performed. The postoperative period was complicated by significant pain and an episode of shock after
autotransfusion. One year later, an analogous procedure was performed on the left hip. Recovery was initially
rapid and uneventful; however, on the third postoperative day a clinical diagnosis of sepsis was made according to
the contemporaneous documentation. Blood cultures and a wound swab were obtained and antibiotic therapy
was subsequently culture‐guided, but the identity of the organism can no longer be retrieved from the records
remaining with the author. As of 2026, at the age of 74, the patient remains physically active.
Conclusions
Despite severe early complications, long‐term functional outcome may be very good. Proper patient selection
and rapid management of complications are of key importance.

Opisany przypadek jest ciekawym przykładem klinicznym potwierdzającym zasadność zastosowania leczenia operacyjnego — kapoplastyki stawów biodrowych — u odpowiednio zakwalifikowanych pacjentów. Przedstawiona wieloletnia obserwacja pokazuje, że mimo powikłanego przebiegu okołooperacyjnego możliwe jest uzyskanie dobrego odległego efektu funkcjonalnego oraz powrót do aktywnego trybu życia, w tym do amatorskiego uprawiania sportu.
