Streszczenie

Wstęp

Urazy pęcherzyka żółciowego są rzadkie, występują u 2.1‐3.5% pacjentów poddanych laparotomii z powodu obrażeń jamy brzusznej [1, 2]. Tak rzadkie występowanie tego uszkodzenia związane jest z lokalizacją tego organu, który leżąc bezpośrednio pod powierzchnią wątroby, jest otoczony również siecią oraz chroniony kostnymi elementami klatki piersiowej [3]. Prawie wszystkie (97‐100%) urazy pęcherzyka żółciowego związane są z uszkodzeniem innych narządów jamy brzusznej [1, 2, 4]. Najczęściej dotyczą one wątroby (83‐91%), w dalszej kolejności dwunastnicy i śledziony. Izolowane urazowe uszkodzenie pęcherzyka żółciowego należy do rzadkości. Dotychczas opisano w literaturze niewiele ponad 50 przypadków [3].

Opis przypadku

33‐letni mężczyzna został przyjęty do SOR jako ofiara wypadku komunikacyjnego. USG jamy brzusznej wykazało zmianę w lewym płacie wątroby budzącą podejrzenie pęknięcia tego narządu. W TK wykazano obecność wolnego płynu w jamie otrzewnej bez cech uszkodzenia wątroby i śledziony. Podczas laparoskopii zaobserwowano obecność krwi oraz żółci w jamie otrzewnej i dokonano konwersji. Podczas operacji uwidoczniono perforację pęcherzyka żółciowego i wykonano cholecystektomię. W jamie brzusznej nie stwierdzono obecności krwawienia z innych źródeł krwawienia oraz uszkodzenia innych narządów.

Wnioski

Izolowane urazy pęcherzyka żółciowego stanowią ogromne wyzwanie diagnostyczne, ze względu na rzadkie występowanie i nieswoiste objawy. W wątpliwych przypadkach wykonanie laparoskopii lub laparotomii zwiadowczej pozwala na potwierdzenie rozpoznania oraz przeprowadzenie interwencji, jaką jest w większości przypadków cholecystektomia.

Abstract

Introduction

Gallbladder injuries are rare, occurring in 2.1-3.5% of of patients undergoing laparotomy for abdominal injuries abdomen [1, 2]. Such a rare occurrence of this injury is related to the location of this organ, which, lying directly under the surface of the liver, is also surrounded by a network of and protected by the bony elements of the thorax [3]. Almost all (97-100%) injuries to the gallbladder are associated with damage to other abdominal organs [1, 2, 4]. The liver is most commonly involved (83-91%), followed by the followed by the duodenum and spleen. Isolated traumatic injury to the gallbladder is rare. To date, just over 50 cases have been described in the literature [3].

Case Report

A 33-year-old man was admitted to the ED as the victim of a traffic accident. An ultrasound of the abdominal cavity showed a lesion in the left lobe of the liver raising suspicion of rupture of this organ. A CT scan showed free fluid in the peritoneal cavity without signs of liver or splenic injury. During laparoscopy, the presence of blood and bile in the peritoneal cavity and a conversion was performed. During surgery, gallbladder perforation was visualized and cholecystectomy was performed. In the abdominal cavity, there was no evidence of bleeding from other sources of bleeding and damage to other organs.

Conclusions

Isolated gallbladder injuries pose a huge diagnostic challenge due to their rare occurrence and non-specific symptoms. In doubtful cases, performing a laparoscopy or exploratory laparotomy allows confirmation of the diagnosis and intervention, which in most cases is a cholecystectomy.
