Streszczenie

Wstęp

Choroba Pageta jest rzadkim nowotworem skóry. Wyróżnia się postać sutkową (ang. mammary Paget disease, MPD) i pozasutkową (ang. extramammary Paget disease, EMPD). Podstawową metodę leczenia stanowi radykalne leczenie chirurgiczne.

Opis przypadku

68-letnia kobieta zgłosiła się z powodu zlokalizowanych na skórze sromu i krocza zmian rumieniowo-łuszczących z towarzyszącym uczuciem świądu i pieczenia. Rozpoznano EMPD, a zmiany usunięto chirurgicznie. Od 2005 r. odnotowano liczne wznowy nowotworu, który ostatecznie objął odbyt oraz skórę lewego pośladka. Każdorazowo leczeniem z wyboru była resekcja chirurgiczna. W 2018 r., ze względu na ryzyko wyłonienia stomii jelitowej, chora nie wyraziła zgody na leczenie operacyjne. Wśród alternatywnych metod leczenia zaakceptowała radioterapię. Zastosowano radioterapię w technice konformalnej VMAT/RapidArc do dawki całkowitej 46 Gy w 23 frakcjach. Do dnia dzisiejszego nie stwierdzono cech wznowy miejscowej EMPD.

Wnioski

EMPD wiąże się z dużym ryzykiem nawrotów choroby. W przypadkach zaawansowanych, nieoperacyjnych lub przy braku zgody pacjenta, można rozważyć radioterapię jako metodę alternatywną dla zabiegów operacyjnych.

Abstract

Introduction

Paget’s disease is a rare skin cancer. Two types of Paget’s disease can be distinguished: mammary Paget disease (MPD) and extramammary Paget disease (EMPD). Radical surgery is a primary method of treatment.

Case Report

A 68-year-old woman reported for erythematous-scaly lesions located on the skin of the vulva and perineum with a feeling of itching and burning. Diagnose of EMPD was established and lesions were surgically removed. Since 2005, there have been numerous recurrences of the disease, which affected the anus and the skin of the left buttock finally. Every relapse was treated surgically. In 2018, due to a high risk of colostomy, patient refused radical surgical treatment. Among alternative methods of dealing, patient accepted radiation therapy. Radiotherapy in the VMAT/RapidArc conformal technique was used with a total dose of 46 Gy in 23 fractions. Until today, no evidence for local recurrence of EMPD has been found.

Conclusions

EMPD is associated with a high risk of recurrences. Radiotherapy may be considered as an alternative to surgery in advanced, inoperable cases or lack of patient’s consent.
