Streszczenie

Wstęp

Przypadek 59-letniego mężczyzny chorego na synchronicznie występujące pierwotne raki obu płuc o jednakowym typie histopatologicznym. Przypadek jest ciekawy ze względu na rzadkość występowania, problemy związane z diagnostyką, z postawieniem rozpoznania synchronicznego wystąpienia dwóch ognisk raka pierwotnego i podjęciem decyzji o sposobie leczenia.

Opis przypadku

59-letni mężczyzna, emerytowany górnik, wieloletni palacz tytoniu zgłosił się w drugiej połowie grudnia 2011 r. do lekarza rodzinnego z objawami zapalenia oskrzeli, suchym kaszlem, ze stanem podgorączkowym. Zbierając wywiad stwierdzono u pacjenta znaczną utratę masy ciała (8 kg w ciągu 2 miesięcy) oraz przeszłość chorobową (stan po cholecystektomii z powodu ostrego zapalenia pęcherzyka żółciowego 2005 r., żylaki podudzi, cukrzyca typu 2).

Wnioski

 W przypadku współwystępowania dwóch zmian nowotworowych w płucach należy zawsze brać pod uwagę możliwość występowania dwóch pierwotnych raków płuca oraz możliwość radykalnego wyleczenia chorego.  Po wykluczenia przerzutów odległych, rozsiewu oraz przerzutów do węzłów chłonnych śródpiersia (PET-CT, mediastinoskopia) pacjenci mogą być leczeni operacyjnie.  Kryteria Martiniego i Melameda są pomocne w rozpoznaniu synchronicznych raków płuca. Chemioterapia adiuwantowa u chorych po resekcji synchronicznych raków płuca przedłuża życie.

Abstract

Introduction

The case of a 59-year-old man suffering from synchronous occurring primary carcinomas of both lungs of identical type histopathological type. The case is interesting because of of the rarity of the occurrence, the problems associated with the diagnosis, with making the diagnosis of synchronous occurrence of of two foci of primary carcinoma and decision-making on the course of treatment.

Case Report

A 59-year-old man, a retired miner and long-time smoker of He reported in the second half of December 2011 to his family doctor family doctor with symptoms of bronchitis, dry cough, with a subfebrile condition. Taking the patient's history, it was found patient's significant weight loss (8 kg in 2 months) and past medical history (status post cholecystectomy due to acute cholecystitis 2005, varicose veins of the lower extremities, type 2 diabetes).

Conclusions

In the case of co-occurrence of two neoplastic lesions in the lungs, the following should always be taken into account The possibility of two primary lung cancers and the possibility of radical cure of the patient. After ruling out distant metastasis, dissemination and metastasis to the mediastinal lymph nodes (PET-CT, mediastinoscopy) patients can be treated with surgery. The Martini and Melamed criteria are helpful in the diagnosis of synchronous lung cancers. Adjuvant chemotherapy in patients after resection of synchronous cancers lung prolongs life.
