Streszczenie

Wstęp

Fibroblastoma desmoplasticum (collagenous fibroma) jest łagodnym guzem tkanek miękkich. Opisany został po raz pierwszy w 1995 roku przez Evansa i wsp. [1]. Guz znany jest również pod inną nazwą ‐ collagenous fibroma ‐ wprowadzoną w 1996 roku przez Nielsena i wsp., zdaniem jej autorów lepiej odzwierciedlającą utkanie histologiczne i łagodny charakter guza [2]. Obie nazwy stosowane są zamiennie. W anglojęzycznej literaturze do tej pory opisano około 120 przypadków fibroblastoma desmoplasticum. Guz ten występu‐ je dwa i pół razy częściej u mężczyzn niż u kobiet [3]. Opisywany był u osób w każdym przedziale wiekowym, najczęściej jednak między 40. a 60. rokiem życia (mediana 50 lat) [4, 5]. W większości przypadków badacze opisywali go jako niebolesny, sprężysty, dobrze odgraniczony guz umiejscowiony w tkance podskórnej lub ‐ rzadziej ‐ w obrębie mięśni poprzecznie prążkowanych, skóry, ścięgien i rozścięgien [6]. Około 1/4 guzów dotyczy mięśni szkieletowych [5].

Opis przypadku

21–letni mężczyzna został przyjęty do kliniki celem diagnostyki przewlekłej, uogólnionej limfadenopatii, z towarzyszącym stanem podgorączkowym. W wywiadzie podał występowanie powiększającego się od paru miesięcy niebolesnego guza na przyśrodkowej stronie lewego uda. Palpacyjnie, w obrębie mięśni przyśrodkowej części uda, stwierdzono obecność sprężystego guza o wymiarach 17x22x22 mm, nieprzesuwalnego względem podłoża. W USG wykazano litą, niejednorodnie hipoechogenną zmianę ogniskową o gładkich obrysach i własnym unaczynieniu, bez cech naciekania otoczenia. Usunięto guz z marginesem przylegającego mięśnia i przekazano do badania histopatologicznego. Na podstawie obrazu mikroskopowego rozpoznano fibroblastoma desmoplasticum.

Wnioski

Należy różnicować fibroblastoma desmoplasticum z guzami tkanek miękkich o gorszym rokowaniu.

Abstract

Introduction

Fibroblastoma desmoplasticum (collagenous fibroma) is a benign soft tissue tumor. It was first described in 1995 by Evans et al [1]. The tumor is also known by another name - collagenous fibroma - introduced in 1996 by Nielsen et al. which, according to its authors, better reflects the histologic texture and benign nature of the tumor [2]. Both names are used interchangeably. In the English-language literature, some 120 cases of fibroblastoma desmoplasticum. This tumor occurs- them two and a half times more often in men than in women [3]. It has been described It has been reported in patients of all age ranges, but most commonly between the ages of 40 and 60 (median age 50) [4, 5]. In most cases, researchers have described it as a non-painful, resilient, well-demarcated tumor located in subcutaneous tissue or, less commonly, in the striated muscles, skin, tendons and tendons [6]. About 1/4 of the tumors involve skeletal muscles [5].

Case Report

A 21-year-old man was admitted to the clinic for the diagnosis of chronic generalized lymphadenopathy, accompanied by a subfebrile state. He gave a history of a lymphadenopathy that had been enlarging for several months non-painful tumor on the medial side of the left thigh. Palpation, within the muscles of the medial part of the thigh, a resilient tumor measuring 17x22x22 mm, immovable relative to the base, was found. Ultrasound showed a solid, heterogeneously hypoechoic focal lesion with smooth contours and its own vascularization, without features of surrounding infiltration. The tumor was removed with a margin of adjacent muscle and submitted for histopathological examination. Based on the microscopic image, the diagnosis was fibroblastoma desmoplasticum.

Conclusions

Fibroblastoma desmoplasticum should be differentiated from soft tissue tumors with a worse prognosis.

Fibroblastoma desmoplasticum należy do grupy rzadkich guzów tkanki łącznej. Ze względu na budowę histologiczną zaliczany jest do guzów o niskiej komórkowości, o dość obfitych pokładach kolagenu, a klinicznie klasyfikowany jako zmiana o łagodnym przebiegu klinicznym. Jedną z cech jest możliwość przerastania tkanek okolicznych, w tym także mięśni. Może to powodować z jednej strony trudność w całkowitym usunięciu zmiany, z drugiej strony nawrót choroby. Ze względu na rzadkie rozpoznawanie, rzeczywista częstość występowania nie jest znana. Oczywiście, możliwość roz‐ poznania zależy od znajomości tej jednostki oraz świadomości występowania rozrostów poza klasyczną lokalizacją, a także u pacjentów poza klasycznie opisaną grupą wiekową. Prezentowany opis przypadku z prezentowanym w dyskusji przeglądem literatury jest dobrym przykładem przedstawienia historii klinicznej a jednocześnie wskazaniem na możliwość wystąpienia u jednego pacjenta więcej niż jednej jednostki chorobowej.
